RULES & REGULATION

CANDIDATE EAGER TO SEEK ADMISSION AND/OR THE STUDENT SHALL HAVE TO SATISFY THE FOLLOWING CONDITIONS

1.   THE CANDIDATES FOR ADMISSION MUST PRODUCE THE ATTESTED COPIES OF CERTIFICATE AND MARK-SHEET OF EXAMINATION PASSED WITH APPLICATION FOR ADMISSION ALONG WITH A CERTIFICATE OF GOOD MORAL CONDUCT.

2.    THE PRESIDENT/DIRECTOR WILL BE THE SOLE AND THE SUPREME AUTHORITY AND HIS DECISION WILL BE FINAL AN THE MATTER OF SELECTION FOR ADMISSION /ADMINISTRATION DECLARATION OF HOLIDAYS/OR OF SUDDEN ANY INCIDENTAL OR ACCIDENTAL CAUSE SHAPPIRINGS/OR REGARDING TEACHING THE STUDENTS OR WAY OF TEACHING.

3.    ADMISSION OF ANY CANDIDATES MAY BE CANCELLED IF REQUISITE PARTICULARS FOUND BE UNSATISFACTORY OR IF HE MIS BEHAVES WITH THE PRINCIPAL/DIRECTOR/CO-ORDINATE WITH THE GUEST LECTURES OF STAFF BUT THE DEPOSITED FEES WILL BE NOT REFUNDED AT ANY COST.

4.    ANY OTHER DECISION TAKEN BY THE PRINCIPAL/DIRECTOR/CO-ORDINATE WILL BE FIND ANY AND THAT CHALLENGE ABLE IN ANY PLACE EXCEPT BEFORE THE PRESIDENT/DIRECTOR OF THE INSTITUTE. ONLY THE PRESIDENT/DIRECTOR CAN TAKE DECISION WHICH CAN NOT BE CHALLENGED IN ANY PLACE.

5.    ANY TYPE OF FEES, I.E. TUITION FEES AND OTHER FEES ONCE PAID WILL NOT BE REFUNDED OR ADJUSTED AT ANY COST. ANY TYPE OF CLAIM OF THE STUDENTS OR OF THEIR GUARDIANS WILL NOT BE ACCEPTED AGAINST THE DIRECTOR/PRINCIPAL/COLLEGE/INSTITUTE ON ANY GROUND AT ANY PLACE.

6.    THE NAME OF THE STUDENT WILL BE STRUCK OFF FROM THE COLLEGE/INSTITUTE LIST IN CASE OF FAILINGS TO PAY THE FEES TILL TEN DAYS AFTER THE SCHEDULED DATE AND HE MAY BE RE-ADMISSION ON PAYMENT OF RE-ADMISSION FEES AND OTHER DUES IF THE PRINCIPAL/DIRECTOR/CO-ORDINATE IS SATISFIED.

7.    POLITICAL ACTIVITIES/UNION MAKING ARE STRICTLY PROHIBITED WITH IN THE INSTITUTE/COLLEGE/HOSPITAL OR HOSTEL PREMISE OR THE STUDENT ARE STRICTLY PROHIBITED TO DO MISBEHAVE, MIS-CONDUCT WITH THE DIRECTOR/PRINCIPAL/LECTURER/STAFF OR WITH THE STUDENT FOR WITH THE PRINCIPAL /DIRECTOR,RAGGING IS STRICTLY PROHIBITED IN ANY CASE.

8.   STUDENT WILL BE LIABLE TO DISCIPLINARY ACTION OR EXPULSION FROM THE College/INSTITUTE. IF ANY REFERENCE MADE BY THEM IN THEM FORMS OR TESTIMONIALS IS FOUND FALSE OR IF THEY DO UNDERSIDE ABLE BEHAVIOUR TO THE DISSATISFY-FACTION OF THE INSTITUTE/COLLEGE OR THE AUTHORITY OF THIS SAID COLLEGE/INSTITUTE OR THEY ACT DETRIMENTAL TO THE INTERESTED OF THE INSTITUTION.

9.    TEST EXAMINATION WILL BE HELD BEFORE ON MONTH ON THE COMMENCEMENT OF THE ANNUAL EXAMINATION. THE DATE WILL BE FIXED BY THE DIRECTOR/PRESIDENT OF THE INSTITUTE (HEAD OFFICE).

10.  THE STUDENT SHOULD HAVE THE ATTENDANCE NOT LESS THEN 75% AT LECTURES,PRACTICAL,DEMONSTRATION AND CLINICAL CLASS, WHICH IS NECESSARY TO APPEAR AT ANNUAL EXAMINATION.

11.   DISCIPLINE,PEACE,WELL MANNER MUST BE MAINTAINED BY THE STUDENT AT ANY PLACE SPECIAL WITH THE DIRECTOR/PRINCIPAL/CO-ORDINATE/STAFF OF THE INSTITUTE/COLLEGE.

12.  ALL COLLEGE/INSTITUTE DUES MUST BE PAID IN TWO INSTALMENT,ON PAYMENT OF FULL AMOUNT ONLY THE (DD) PUPILS WILL BE ABLE TO APPEAR AT THE EXAMINATION OR THE DIRECTOR -IN-CHARGE AS FIT AND PROPER.

13.   ""NO DUES CERTIFICATE" MUST BE PRODUCED BEFORE TAKING EXAMINATION FORM AND IT WILL BE ISSUED ONLY ON RECOVERY OF THE COLLEGE/INSTITUTE DUES FINES AND BOOK ETC.IT IS TO BE NOTED SINCERELY THAT THE ANNUAL EXAMINATION FEES WILL BE GIVEN TO THE INSTITUTE (H.O.) BY THE STUDENTS AS PER THE INSTRUCTION OF THE PRESIDENT/DIRECTOR EXCLUDING THE COLLEGE/INSTITUTE FEES.

14.   THE PRINCIPAL/CO-ORDINATE OF THE COLLEGE/INSTITUTE IS BOUND TO ABIDE BY THE RULES AND REGULATIONS OF THE SAID HEAD OFFICE(INSTITUTE).

15.   THE STUDENT HAS TO PAY ALL THESE FEES ACCORDINGLY DECLARED BY THE COLLEGE/INSTITUTE/HEAD OFFICE. THE STUDENT SHOULD PAY THE EXAMINATION FEES FIXED UP BY THE PRESIDENT/DIRECTOR OF HEAD OFFICE.

16.   THE INSTITUTE HAS LIMITED SEATS. AFTER FILLING UP THE SEAT EXTRA CANDIDATE CAN NOT BE ALLOWED FOR ADMISSION.

17.   THE INSTITUTE WILL NOT BE REFUND ANY KIND OF FEES DEPOSITED IN ANY CHANGE IN CIRCUMSTANCES OR FAMILY ECONOMIC CONDITIONS OR CONFLICTS OR DECISION TO DISCONTINUE THE STUDY OR ANY OTHER REASONS.

18.   EXAMINATION FEES WILL BE CHARGED SEPARATELY AS FIXED BY HEAD OFFICE.


IMPORTANT INSTRUCTION

1.   Understand properly to recognition, admission schedule, fee, instruction provided on the council portal before filling the admission form.

2.   Information regarding the Eligibility rules, courses , subjects, fee structure, free ship and scholarship etc. is available on the council portal. It is a applicant's responsibility to cheque all the information before taking the admission to any courses.

3.   In case of Free ship and Scholarship if the applicants are failed to submit/produce any of the required document he will not be able to claim for Free ship and scholarship. CCAM will not accept the forms of those applicant.

4.   Applicant's applying for admission should be present in person at nearest Admission office or H.O. at the time of filling the admission form.

5.   Applicants can also download the application form through the website.

6.   Fill it with black ball pen only. It is expected that application form should be clearly and completely filled . Avoid overwriting the letters.

7.   For the purpose of postal communication applicants are requested to fill his complete postal address in application form. Board will not be responsible for postal communication in case of address in wrong.

8.   Applicant's are requested to select the subjects carefully as the selected subject will appear on Admit Card.

9.   Fee should be paid by only Demand Draft. Fees once paid should not be refunded or not adjustable or changed under any rule.

10.   Demand Draft should be drawn in the favour of CHHATTISGARH COUNCIL OF ALTERNATIVE MEDICINE and it should be payable at lormi (c.g.).

11.   Attached all necessary original and attested true copies of the certificates with the application form.

12.   Affix a recent passport size photograph in the appropriate box provided in the application form.

13.   Candidates are requested to verify all the necessary document as per the board's requirement and do not accept the form with incomplete document in any case.

14.   Admission form filling is not a final status of admission confirm. All the application form submitted at council will be checked and verified by the council administration section.

15.   After verification of document submitted by the applicants, council will issue Enrollment Number to the eligible student only. The EN will be Communicated to applicants through SMS and Email address provided by the applicants in the application form.

16.   The Council/organization run non-formal self employment education programme only and not give any guarantee for further educational use or any job.



ADMISSION FORM AND PROSPECTUS

THE ADMISSION FORM AND PROSPECTUS CAN BE OBTAINED ORM THE INSTITUTE DIRECTLY ON PAYMENT OF RS.350/- OR BY POST BY SENDING M.O./DD. FOR RS.550/- IN FAVOUR OF INSTITUTE AT BILASPUR THE CANDIDATES MUST FILL FROM IN HIS OWN HANDWRITING AND SHALL SUBMIT THE SAM IN THE OFFICE OF THE PRINCIPAL/DIRECTOR/CO-ORDINATE WITH REQUIRED DOCUMENT AND PRESCRIBED FEES.

ON CONFORMATION FOR ADMISSION IN ANY COURSE,THE APPLICATION FOR ADMISSION SHOULD ACCOMPANY THE FOLLOWING NECESSARY/DOCUMENTS.

1.    APPLICATION FORM IN DUPLICATE (AS ATTACHED WITH PROSPECTUS)

2.    TWO PHOTO-COPY OF MARK-SHEET(10th & 12th) OF ACADEMIC EDUCATION DULLY ATTESTED.

3.    RESIDENTIAL PROOF ATTESTED COPY

4.    3 NO. COLOR PASSPORT SIZE PHOTOGRAPH

5.    CHARACTER CERTIFICATE FROM THE SCHOOL/COLLEGE LAST ATTENDED BY THE STUDENT OR FROM A GAZETTED OFFICER.

6.      ORIGINAL DOCUMENT MUST BE PRODUCED WHEN REQUIRED.
